What to Do and What Not to Do After a Filler Treatment
Hyaluronic acid filler applied to your face (whether lip filler, (whether on the cheeks, chin, or under the eyes), the gel has a soft, gel-like consistency at the time of the procedure—it has not yet hardened or integrated into your skin. It takes an average of 10–14 days for this gel to blend with your skin’s natural texture and retain its shape.
During this delicate process, to avoid jeopardizing your aesthetic investment and to maintain symmetry, After the filling Points to Keep in Mind You must follow the instructions exactly. Even the slightest mistake could double the cost or cause the filler to shift to an unintended area.
Understanding the rationale behind these rules also answers other frequently asked questions. What you should realistically expect from the process Expectations Before and After Filler Treatment In my article, When is it okay to give a facial massage? I'll explain this in a separate section. You can do this at home skin care steps, duration of persistence and possible side effects I wrote about it in detail. The First 24–48 Hours: A Critical Waiting Period
On the day of your procedure and the following day, the following guidelines apply to help the filler retain its shape and prevent infection.
- Never give a massage: Unless your doctor specifically tells you to “rub this area,” do not apply pressure to the area, do not touch it, and do not rub it. Even drying your face vigorously with a towel while washing it can cause that soft filler to shift.
- Makeup is strictly prohibited: The needle entry points (even if they are microscopic) remain open for the first 24 hours. Bacteria entering through your makeup sponge or foundation can lead to a serious abscess infection.
- High pillow: Keeping your head higher than your heart while sleeping (using two pillows) helps prevent morning puffiness and swelling around the eyes. Avoid sleeping on your stomach (which compresses the tissue) for at least 3–4 days.
- No heat or steam: During the first 48 hours, very hot baths, Turkish baths, saunas, and tanning beds can significantly increase swelling and bruising because they cause blood vessels to dilate.
A more specific procedure performed primarily around the nose and lips, nasolabial filler The rules above apply exactly the same way in this case as well.
Medication Use and Other Procedures
You should not use pain relievers or creams on your own without your doctor’s approval. The table below summarizes what you need to be aware of during this period.
| Dos and Don'ts | Rationale |
|---|---|
| Aspirin and Blood Thinners | Do not take this medication without your doctor’s knowledge during the first few days, as it may increase bleeding at injection sites or cause subcutaneous bruising (ecchymosis). |
| Sports and Intense Exercise | For the first 2–3 days, you should avoid exercises that raise your blood pressure and heart rate. High blood pressure causes swelling. |
| Drink Plenty of Water | YOU MUST DO THIS! Hyaluronic acid is a substance that gains volume by drawing in water. Drinking at least 2.5 liters of water a day improves the quality of the filler. |
Published to prevent complications following dermatological procedures U.S. Food and Drug Administration (FDA) Their manuals also emphasize that post-procedure care is just as important as the quality of the materials used.
Methods for Reducing Swelling After Filler Injections
Because hyaluronic acid retains water, it is normal to experience mild swelling and edema during the first 24–48 hours after the procedure. The most effective After the filling A method for reducing swelling is to apply a cold compress to the treated area at the frequency recommended by your doctor (usually 10 minutes every hour).
In addition, drinking plenty of water helps speed up lymphatic flow, allowing edema to subside more quickly. Avoiding a high-sodium diet and alcohol also helps reduce swelling. Thanks to these simple measures, the swelling subsides noticeably within a few days, and the lips or treated area quickly return to their natural appearance.
What Should the Sleeping Position Be After a Filler Treatment?
For the first 3 nights, you should follow the post-filler sleeping position guidelines to prevent the filler from shifting and to avoid pressure-related asymmetries. The best approach is to sleep on your back with your head elevated above the rest of your body (using two pillows).
This position also helps the fluid that may accumulate in the face to drain quickly. You should definitely not lie face down; do not allow the pillow to come into contact with the treated area—whether it’s the cheek, temple, or chin. Sleeping on your side is also strongly discouraged during this sensitive period, as it can cause the filler to shift to one side.
Guidelines for Hyaluronic Acid Fillers
At our clinic, we offer our patients hyaluronic acid filler These guidelines are designed to protect the skin during the first week. For the first 24 hours, avoid getting the face wet, do not apply makeup, and do not touch the area with dirty hands to prevent the risk of infection.
You should avoid environments such as saunas, Turkish baths, hot baths, and tanning beds—which dilate blood vessels and increase the risk of swelling and bleeding—for at least one week. Dental procedures should also be scheduled at least two weeks after a filling, as they can cause excessive strain on the teeth. These clear and scientifically based guidelines ensure both the durability and safety of the treatment.

Filler Massage: When and How?
Filler massage is a procedure that should only be performed when your doctor deems it necessary and exactly as they instruct. Massaging or applying pressure to the filler on your own can cause the material, which has not yet settled, to shift and result in deformities.
For this reason, you should absolutely avoid unguided massage during the first two weeks. If you feel even a slight lump or hardness, you should show it to your doctor during your follow-up appointment rather than trying to treat it yourself. If necessary, we will either perform the correct massage technique on you ourselves or explain how to do it. In short, remember: during this sensitive period, the “best massage” is actually not touching the filler at all.
Restrictions on Dental Treatment and Skin Care
For the first 2 weeks following a filler treatment, you should avoid dental procedures, deep dental cleanings, and aggressive skin care treatments. Excessive opening of the mouth during a dental examination or the use of strong suction during skin care treatments can disrupt the tissue plane where the filler is injected, leading to displacement.
For this reason, the safest course of action from a medical standpoint is to either have your scheduled dental and skin care procedures done before the filling or to postpone them until at least 15 days afterward. It is best to schedule all non-emergency invasive procedures after waiting for the filling to fully set.
Hygiene and Makeup Restrictions
Until the micro-pores created by the needle have healed (at least 24 hours), do not allow a makeup brush, sponge, or cosmetic powder to come into contact with the treated area. Bacteria that may accumulate on these items can enter through the open pores, increasing the risk of infection.
On the first day, you should cleanse your skin only with sterile saline solution or a cleansing solution recommended by your doctor. This ensures that the healing process is completed successfully without any risk of infection. A Brief Summary for the First Week After Filler Treatment
The care you take during the first week directly determines how long-lasting the results will be. Avoiding pressure on the area and keeping your head upright on the first day helps reduce swelling. It is safest to avoid strenuous exercise, hot environments, and alcohol for the first 48 hours.
In the days that follow, simply drink plenty of water and follow the aftercare instructions provided by your doctor. However, if you experience unexpected pain, excessive swelling, or a change in color, you should contact your clinic immediately.
The Impact of Nutrition and Habits on Recovery
Even small dietary choices during the post-operative period can affect the rate of recovery. Foods that are too salty increase water retention in the body, making edema more pronounced; therefore, it is helpful to reduce salt intake during the first few days.
Alcohol should not be consumed during the first 24–48 hours because it both dilates blood vessels and disrupts fluid balance. Smoking, on the other hand, negatively affects circulation and slows down healing. In contrast, drinking plenty of water, eating a balanced diet, and consuming foods rich in vitamin C help the tissue recover more quickly. In other words, the quality of the outcome is determined not only by what happens at the clinic but also by the choices you make at home.
Distinguishing Between Normal Symptoms and Warning Signs
Mild swelling, tenderness, small bruises, and tiny lumps that can be felt when touched are completely normal after the procedure. These symptoms usually subside on their own within a few days and are no cause for concern.
However, symptoms such as progressively worsening severe pain, swelling that becomes more pronounced on one side, and a sensation of coldness accompanied by whitening or bruising of the skin are different. Although rare, these may indicate a vascular problem and require immediate medical attention. That is why our clinic provides patients with a contact number they can reach after their procedure. Knowing in advance what is normal and what constitutes a warning sign will help you go through this process with much greater peace of mind.
