Botox Brands: The Difference Between Botox, Dysport, and Xeomin

Most of the clients who come to my clinic think the word “Botox” is the name of the procedure. In fact, Botox is a brand name; the procedure itself is the application of botulinum toxin. There are several different brands on the market that contain the same active ingredient, and which one is used affects not the outcome itself, but how the procedure is planned.

Is Botox a brand name or the active ingredient?

Botox is the brand name of a product manufactured by Allergan. In everyday Turkish, it has come to be used—much like “Selpak”—to refer to the entire category. The medical term for the procedure, however, is “botulinum toxin type A treatment.”.

Knowing this distinction is useful in practice: when a clinic says, “We offer Botox,” that doesn’t specify which product is being used. You have the right to ask which brand and what dosage is being used.

What is botulinum toxin, and how does it work?

Botulinum toxin is a protein derived from the Clostridium botulinum bacterium and purified under laboratory conditions. It is used in very low doses in cosmetic procedures.

Here’s how it works: It temporarily blocks the release of acetylcholine—the neurotransmitter that carries the “contract” command from the nerve ending to the muscle. Since the signal does not reach the muscle, it relaxes, the overlying skin is relieved from constant contraction, and the line softens. Its classification as a neurotoxin stems from the fact that it targets this nerve-muscle communication; a systemic effect is not intended, and it remains localized to the treated area.

The main difference between Botox, Dysport, and Xeomin

All three contain botulinum toxin type A. The difference lies in the carrier protein structure surrounding the molecule.

• Botox (Allergan): The toxin is found in conjunction with complex proteins. This product has the longest history of use.
• Dysport (Ipsen): The structure of complex proteins is different; they have a smaller molecular size and tend to spread out slightly more widely within the tissue.
• Xeomin (Merz): It has been stripped of its complex proteins, which is why it is referred to as a “naked molecule.”.

These differences do not completely change the outcome; they alter the physician’s dose and treatment planning.

Unit difference: Why is the number higher for Dysport?

This is the most common point of confusion. The number of units specified for Dysport is significantly higher than that for Botox, but this does not mean that “more medication was administered.”.

Each brand’s units are defined according to its own measurement system; there is no one-to-one conversion between brands. In other words, 20 units of Botox are not the same as 20 units of Dysport. When making comparisons, one should focus not on the number of units, but on the treatment area and the doctor’s treatment plan. When does the effect begin, and which one is faster?

The effects of all three products are not immediately visible; the process of binding to the nerve endings takes a few days. Generally speaking, it is reported that the first signs appear slightly earlier with Dysport, but the difference is only a matter of a few days and varies from person to person.

What determines which brand is chosen?

The decision depends on the area and the desired outcome. A product that spreads more widely may be advantageous on large, flat areas, whereas a more controlled option is preferred for sensitive areas—such as the eye area—where you don’t want the product to spread to adjacent muscles.

Does changing the brand change the impact?

There is no medical contraindication to using Botox in one session and Xeomin in the next. However, since the units are not directly equivalent between the two brands, the dose must be recalculated when switching.

Is there such a thing as resistance development?

Although rare, a decrease in effectiveness may be observed in some individuals after repeated treatments. One possible explanation for this is that the body develops antibodies against the toxin.

There are two situations believed to increase the risk: repeating the treatment more frequently than necessary and using a high dose at each session. For this reason, not shortening the interval between sessions and not increasing the dose unless necessary is not only an economic choice but also a medical one. Under what circumstances is the implementation postponed?

There are situations in which botulinum toxin treatment is postponed, regardless of the brand. Pregnancy and breastfeeding are at the top of this list. If there is an active infection, an open wound, or a noticeable skin problem in the treatment area, the condition must be allowed to heal first.

The concurrent use of certain neurological disorders affecting the neuromuscular junction and specific classes of antibiotics also requires evaluation. Therefore, it is important that you provide a complete list of the medications you are taking and any known medical conditions during your initial consultation; the decision is based on this information.

How are products selected at the clinic?

In my clinic, I don’t stick to just one brand. The decision depends on three factors: the treatment area, the client’s response to previous treatments, and how noticeable the desired result should be.

For a client receiving Botox for the first time, I generally prefer to start with a more conservative dose; if necessary, we’ll do a touch-up at the follow-up appointment two weeks later. This approach is based on the principle of “starting small and adding more if needed,” and it’s much more comfortable than having to deal with an over-treated result.

What Does the Price Difference Reflect?

There are price differences between brands, and there are two reasons for this. The first is the cost of importing and licensing the product; the second is that the unit definition differs.

The second point is often misunderstood. If a brand’s unit price appears lower, the number of units required for the same area may be higher. The comparison should be based on the total cost per area to be treated, rather than per unit. Otherwise, the option that seems cheaper may end up costing more overall.

What Products Are Used in Turkey?

In Turkey, only products licensed by the Ministry of Health are used for cosmetic botulinum toxin treatments. The use of unlicensed products brought into the country privately from abroad is problematic from both a legal and medical standpoint.

In practical terms, this means that the packaging of the product to be used must include a Turkish package insert and licensing information. Requesting this information is not a sign of skepticism; it is a fundamental patient right.

Which One Should You Start With for Your First Application?

For someone getting Botox for the first time, the choice of brand is less important than the dosing strategy. Regardless of which product is used, the safest approach is to start with a conservative dose during the first treatment and reassess after two weeks.

The reason is simple: muscle strength and the body’s response to toxins vary from person to person, and this can only be determined through trial and error. It's Not the Brand, but the Implementer Who Makes the Difference

When my clients focus on the brand, I usually say this: the same product yields two different results when used by two different doctors.

The choice of injection site, the distribution of the dose across different areas, and an accurate understanding of facial anatomy are far more critical than the product itself. For this reason, it is a better investment to devote part of the time spent researching brands to researching the experience of the physician who will perform the procedure.
