There are two procedures that confuse our patients who step into the world of medical aesthetics the most, are used most frequently interchangeably, but are actually as different as night and day: Botox and Filling.
What are the Differences Between Botox and Fillers? Get to Know Two Giants of the Aesthetic World
Many of my clients who come to my clinic may say, “I want Botox for my lips” to plump up their lips or “Will it go away if we put some filler there?” for the deep hollows on their forehead. This confusion is quite natural because both procedures are injectable, both target wrinkles and both promise rejuvenation. However, their biological working mechanisms, targets, contents and lifespans are completely different. Performing the wrong procedure on the wrong area can lead to unwanted aesthetic results. "I want Botox for my lip": The Most Common Misunderstanding
First of all, let's start by breaking this myth: Botox does not swell the lip. Botox does not give volume. Botox is not a “filler”. If you want your lips, cheeks or chin to grow and plump up, what you need is a filler, not Botox. Botox is a drug that only affects the muscles; filler is a physical gel. Making this distinction is the first step in shaping your expectations from the treatment correctly.
Basic Working Principles: One Freezes, the Other Fills
To understand the fundamental difference between these two processes, we need to look at “how they work”.
Botox (Neurotoxin): Relaxing and Ironing Muscle
Botox (Botulinum Toxin), Clostridium botulinum is a purified protein derived from bacteria. Its working principle is “blockade”. It binds to the nerve endings in the area where it is injected and temporarily stops the “contract” command from the nerve to the muscle. Without the command, the muscle relaxes and cannot move. When the muscle does not move, the skin on it does not fold and wrinkle. In other words, Botox temporarily eliminates the cause (muscle movement) that causes the problem (wrinkles). It is kind of like ironing a shirt; it stretches and smooths the fabric.
Dermal Filler (Filler): Volumizing and Contouring
Dermal Fillers as the name suggests, it performs “filling”. It is usually a Hyaluronic Acid-based, gel-like substance. When injected under the skin, it occupies a physical space. It lifts up a sunken area (e.g. under the eyes or temple), fills an empty area (cheeks) or gives volume to a thin lip. The filler does not affect the muscles; the muscles continue to work underneath, while the filler pushes up and tightens the overlying skin. This is similar to inflating a deflated balloon.
Which one for Which Wrinkle? Dynamic vs. Static Distinction
Wrinkles are divided into two groups and their treatment is determined accordingly. You can test which group you fall into when you look in the mirror.
Dynamic Wrinkles: The Artifact of Mimics (Botox Field)
“Dynamic Wrinkles” are lines that are not visible when your face is still (without facial expressions), but appear when you frown, laugh or are surprised.
- “11” lines in the center of the eyebrow.
- Horizontal lines on the forehead.
- Crow's feet in the corner of the eye.
Since these lines are caused by muscle movement, the treatment is based on Botoxtour. It is generally not preferred to fill this area because if the muscle continues to move, the filling may be displaced.
Static Wrinkles Gravity and Volume Loss (Filling Area)
Do deep grooves or lines appear on your face when you completely relax your face, even if you make no facial expressions? These are “Static Wrinkles”. They are the result of years of collagen loss, gravity and tissue melting.
- Lines descending from the edge of the nose to the mouth (nasolabial).
- Lines descending from the corner of the mouth to the chin (Marionette).
- Depressions in the cheeks.
These lines are now established, they will not open even if you stop the muscle. They need to be physically filled underneath, so the solution is Filling.
Application Regions: Which Area of the Face?
Although there is no general rule, facial anatomy has divided these two procedures into specific regions.
Upper Face Dominance: Forehead and Eye Contour
The upper part of the face (eyebrows, forehead, eye corners) is the area where facial expressions are most intense. Botox is the undisputed master of this area. The use of fillers here is very rare and requires advanced technique.
Mid and Lower Face Dominance: Cheek, Lip and Chin
In the middle and lower part of the face, volume loss and sagging are at the forefront. Fillers are used to define the cheekbones, enlarge the lips, sharpen the jawline or hide the bridge of the nose. The use of Botox on the lower face is limited (only on specific points such as masseter, gingival smile or upper lip barcode lines).
Content Difference: Laboratory Production vs. Natural Ingredients
The chemistry of these two substances is also completely different.
- Botox It is a drug derived from bacteria in a laboratory environment. It is not permanent in the body, it is metabolized and excreted. It is not a building block of the body.
- Filling: The most commonly used fillers today contain “Hyaluronic Acid”. Hyaluronic acid is a sugar molecule found naturally in the human body (eyes, joints, skin). Therefore, fillers are more biologically compatible with the body and they dissolve and disappear over time.
Duration of Effect and Speed of Results
When will you say “it's done” after the appointment?
Botox A Week of Patience
Botox does not work “instantly”. Immediately after the procedure, you will not see any changes in your face. The drug binds to nerve endings and stops the muscle 3 to 7 days for it to set. It takes 14 days to fit completely. Permanence is average 3-6 months.
Filling Instant Change and "Wow" Effect
Filling gives volume in the second it is applied. When you leave the clinic, your cheekbones are elevated or your lips are plumped (even if there is some edema). The result is immediately visible. Permanence depends on the area used and the density of the product. 12 to 18 months between
Safety and Recycling: What Happens If Mistakes Are Made?
Both processes are safe, but the “recycling” schemes are different.
- It is Possible to Melt the Filling: The biggest advantage of hyaluronic acid fillers is that they can be dissolved instantly with an enzyme called “Hyaluronidase” if they are not liked or if there is a problem. So there is an “undo” button.
- Botox is Difficult to Undo: Once Botox has been administered and the muscle has taken hold, you need to wait until the effect of the drug wears off (3-4 months). There is no antidote to destroy Botox instantly; its effect diminishes over time. Botox therefore requires much more precise dosing.

Strength Comes from Unity: Botox and Filler Combination
Many patients have both dynamic lines and volume loss at the same time. In this case, the best result is obtained with “Combination Treatment”. For example, Botox between the eyebrows prevents wrinkling of that area; however, if there is a deep cleft (static scar) between the eyebrows, Botox alone is not enough. A thin filling is applied under that crevice and the skin is smoothed. This is called “Liquid Facelift”. When used together, they increase the life and effect of each other.
