30/10/2025What are the Side Effects of Dermal Fillers? Risks and Safety
Dermal fillers treatments are one of the most popular and effective methods offered by medical aesthetics to address facial volume loss, soften lines and achieve a more youthful, dynamic expression. The use of body-compatible substances such as hyaluronic acid makes this treatment attractive to millions of people. However, as with any medical procedure, being informed about the potential side effects and risks of dermal fillers is the basis of being an informed patient and approaching the process with confidence.
What are the Side Effects of Dermal Fillers? Risks and Safety
“Questions such as ”What can happen to me if I have fillers?“, ”How safe is this procedure?“, ”What if there is a bad result?" are the most natural concerns at the stage of deciding on treatment. As Dr. Melis Ülger, Istanbul, Kadikoy‘Therefore, I would like to explain the possible side effects of dermal fillers, the causes of these risks, and most importantly, what should be done to minimize these risks in this guide with all transparency.
Overall Safety Profile of Hyaluronic Acid Fillers
First of all, it should be noted: The gold standard used today Hyaluronic Acid (HA) based fillers have an extremely safe profile when applied by a specialist physician, with the right technique, with quality and approved products. Since HA is a substance that is already naturally present in our body, the risk of allergic reaction is minimal and is metabolized and excreted by the body naturally over time. In addition, the fact that it can be dissolved with the enzyme “hyaluronidase” when necessary provides an additional layer of safety. However, this high safety profile does not mean that the application is risk-free. Risks are usually related to the application technique, the knowledge and experience of the practitioner and hygiene conditions rather than the product itself.
Expected and Temporary Side Effects: Don't worry, they are normal!
This group are natural and temporary reactions of the body to the procedure, which can be seen to varying degrees in almost every patient after filler injection. They are not complications but a normal part of the healing process.
Swelling (Edema)
Due to the minimal trauma caused by the injection and the water-absorbing properties of hyaluronic acid, it is very normal to see mild to moderate swelling in the treated area within the first 24-72 hours after the procedure. This swelling may be more pronounced especially in sensitive areas such as the lips. Applying cold compresses and keeping the head elevated helps the swelling go down faster. Swelling usually subsides significantly within 3-7 days.
Bruising (Ecchymosis)
Due to the rich network of capillaries on our face, the tip of the needle or cannula may inadvertently hit a small vessel during injection. This can cause a small blood leak and bruising under the skin. The severity of the bruise varies from person to person and according to the application site. It usually changes color within a few days (from purple to green, then yellow) and disappears completely within 1-2 weeks. Avoiding blood thinners before the procedure and applying cold compresses after the procedure reduces the risk of bruising.
Redness and Sensitivity
A slight redness, sensitivity to touch or a slight itching sensation at the injection points are also normal conditions that can be seen in the first few days after the procedure.
Rare but Possible Complications to Be Aware of
This group includes more serious conditions that, with the right precautions, are extremely rare but need to be recognized. The occurrence of these risks is usually associated with poor hygiene, incorrect injection technique or lack of anatomical knowledge on the part of the practitioner.
Infection
As with any injection procedure, there is a risk of infection if performed under non-sterile conditions or without observing hygiene. Symptoms such as increased pain, increased temperature, severe redness and discharge at the application site may be signs of infection and require immediate medical attention.
Clumping or Nodule Formation
The filler does not distribute smoothly under the skin and forms small hardness or lumps (nodules). This can be caused by incorrect product selection (for example, injecting a very dense filler close to the surface) or incorrect injection technique. Early lumps can usually be corrected with massage. Late nodules may sometimes require hyaluronidase injection or other treatments.
Allergic Reactions
True allergic reactions to modern HA fillers are extremely rare. However, sometimes sensitivity may develop to other substances in the filler (e.g. lidocaine) or residues in the manufacturing process. In case of swelling, redness and itching that lasts longer than expected after the application, a physician should be consulted.
Vascular Occlusion: Most Serious Risk
This is the rarest but most serious complication of dermal fillers. This is when the injected filler inadvertently enters a blood vessel or presses on the vessel from the outside, blocking blood flow. This can lead to impaired blood supply to the tissue supplied by the blood vessel and, if left untreated, tissue death (necrosis) or blindness (if a blood vessel around the eye is affected). Symptoms include severe pain during or immediately after the procedure, sudden blanching or bruising of the skin (livedo reticularis pattern). This requires immediate dissolution of the filler with the enzyme hyaluronidase. The existence of this risk demonstrates why it is vital that the procedure is only performed by experienced medical doctors who are highly familiar with facial anatomy.
The Key to Minimizing the Risk of Side Effects: Choosing the Right Specialist and Clinic
The only and most effective way to minimize all the rare but serious risks mentioned above is to have the treatment in the right place, by the right person. A medical doctor knows the vascular and nerve map of the face, can recognize a possible complication early, and most importantly, has the authority and knowledge to use emergency intervention tools such as hyaluronidase when necessary (for example in case of vascular occlusion). Instead of focusing on price when making your choice, focusing on the experience of the physician, the hygiene of the clinic and the quality of the products used will be the best investment for your health. In this regard Organizations such as the American Society of Plastic Surgeons (ASPS) patient safety guidelines also emphasize expert selection.
